Senior Social Worker

The Role

The Head of Community Services, Adult Social Services is inviting applications for this secondment opportunity as a Senior Social Worker within the Community Adult Health Service (CAHS). This is an exciting opportunity for somebody who is keen to take their first steps into a management role and progress their professional development.

You will need to be committed to the development of the CAHS service by working alongside the Assistant Service manager for this team. So that you provide a timely, joined up approach to health and social care, enabling holistic care delivery for patients and carers who would benefit most from this integrated model of care.

You will report directly to the Assistant Service manager for the CAHS team and you will be an active participant of the Operational Management Team. However, you will also need to develop close working relationships with other health professionals from the Clinical Commissioning Group, including GP’s etc.

The successful candidate will be a registered social care professional, who will be able:-

  • To undertake statutory interventions where service users are deemed to be at risk, including situations where people may be neglected or abused in accordance with the Inter-Agency Policy and Procedures for Safeguarding Adults and to chair protection of vulnerable adults’ meetings where required
  • Ensure that the team carries out its statutory functions under the Care Act 2014
  • To provide regular, formal professional supervision for practitioners in order to support staff effectively
  • Have good report writing skills, IT skills and can manage various work streams, within specific time scales.

They should also have a commitment towards strength-based outcomes and working in a multi-disciplinary way with other health professionals.

The CAHS team takes a multi-agency approach, working alongside health colleagues. The work within the team is varied, which includes complex care management, with a strong focus on wellbeing, keeping people in the community, managing safeguarding enquiries and providing direct payments to increase individuals’ control over their own care.
