We are looking for Assistant Service Managers, in our Hospital Social Work teams, who will share our values and vision for integrating health and social care and our discharge to assess services.
These management positions are in our Hospital Social Work teams:
- Richmond Response & Rehabilitation Team (RRRT)
- Wandsworth (St George’s Hospital Team)
- Wandsworth Hospital Team (out of borough)
Richmond Response and Rehabilitation Team (RRRT SW)
RRRT is a well-established and integrated health and social care team, where we work collaboratively to achieve an excellent service for our residents in Richmond. Working across several NHS acute hospital Trusts and community health services, we deliver high quality, effective outcomes though working and supporting each other, within a committed dedicated team.
St Georges Hospital Team (SGH)
Based in Tooting Broadway and is home to one the busiest A&E Departments in Britain, as featured on Channel 4’s, ‘24 Hours in A&E’. Our social work team is at the heart of this discharges within this diverse hospital. We have a pivotal role in supporting Wandsworth residents, their families, and our health partners. We work within a Discharge to Assess Model, across a multi-disciplinary team, to support safe discharges from hospital.
Wandsworth Hospital Team (WHT)
This team serves our Wandsworth residents on their journey out of any hospital that they are admitted to (apart from St George’s Hospital). Working with several of our large London hospitals, we are supportive and have strong well-established links with our NHS colleagues and partners to facilitate safe and timely discharges from various hospital sites.

The Assistant Service Manager role will appeal to someone seeking to enhance their current skills and experience of leading and inspiring a team, with the potential for career progression into senior management. You will be innovative, inspiring and creative in managing our successful teams, helping us to deliver safe and well-co-ordinated hospital discharges. You will oversee the effective management of hospital discharges from acute and non-acute settings and quickly build positive working relationships with our Health and community partners as a natural communicator.
Our service delivers positive outcomes 7 days a week and work often involves responding to a change in circumstances, so the ability to think be quickly, be decisive and act calmly is required. The successful candidates will deliver effective risk management, using their knowledge of legislation and guidance in relation to hospital discharge and continuing health care.
We are looking for:
- An innovative and dynamic social care manager with experience of managing people
- Strong supportive leadership with excellent communication and IT skills
- The ability to embrace change, whilst maintaining quality practice and performance
- The ability to build and maintain positive working relationships with Health partners and organisations
- A commitment to equality diversity and inclusion and resilient approach
- An ability to embed statutory functions under the Care Act 2014 and in accordance with the Inter-Agency Policy and Procedures for Safeguarding Adults

The successful candidate will be a registered social worker with Social Work England.
