We are seeking to welcome Assistant Service Managers, in our Hospital Social Work teams, who will share our values and vision for integrating health and social care and our discharge to assess services.

About the role 

Richmond Response and Rehabilitation Team (RRRT) Social Care

RRRT is a well-established and integrated health and social care team, where we work collaboratively to achieve an excellent service for our residents in Richmond. Working across several NHS acute hospital Trusts and community health services, we deliver high quality, effective outcomes though working and supporting each other, within a committed dedicated team.

The Assistant Service Manager role will appeal to someone seeking to enhance their current skills and experience of leading and inspiring a team. You will be innovative, engaging and creative in managing your team, helping to deliver safe and well-co-ordinated hospital discharges. You will oversee the effective management of hospital discharges from acute and non-acute hospitals and quickly build positive working relationships with our Health and community partners as a natural communicator.

Our service requires managers to be able to respond to a change in circumstances, due to a hospital admission. Therefore, the ability to think quickly, act calmly and to be decisive is necessary for this role. The successful candidate will deliver effective risk management, with a clear knowledge of legislation and guidance in relation to hospital discharge and Continuing Health Care, to promote well-being and positive outcomes.

We are looking for:

  • An innovative and dynamic social care manager with experience of managing people
  • Strong supportive leadership with excellent communication and IT skills
  • The ability to embrace change, whilst maintaining high quality practice and performance
  • The ability to build and maintain positive working relationships with Health partners and external organisations
  • A commitment to equality diversity and inclusion
  • An ability to embed statutory functions under the Care Act 2014 and work in accordance with the Inter-Agency Policy and Procedures for Safeguarding Adults

Essential Qualifications, Skills and Experience

A professional qualification in Social Work or Occupational Therapy and registration with SWE/HCPC.

Experience and skills in managing people

Leadership and Management training/qualification (or willing to undertake)
